The Sexual Healing Journey: A Guide for Survivors of Sexual Abuse
Wendy Maltz.  Harper Collins Publishers, New York, N.Y. 1991.  This book is useful for survivors and especially for sex addicts.  A  number of pencil and paper exercises are included.

The Courage to Heal Workbook: For Women and Men Survivors of Child Sexual Abuse
